2) Insurance Card (if you have insurance)

If you do not have insurance, meet all of the following to qualify for a free mammogram through the Kansas Early Detection Works Program: live in Kansas, are between the ages of 40 to 64, and meet income eligibility for the program. 3) Name of Your Primary Care Provider.
