✅ What to Bring – Enrollment Checklist
To complete an application, please have the following for each person in your household applying for coverage:

• Dates of birth
• Social Security numbers
• Immigration documents (visa, green card, etc.) if applicable
• Most recent tax documents (W-2, 1040, 1099)
• Paystubs or income details (Profit & Loss statement if self-employed)
• Information on current health insurance (if any)
