INFORMATION EVENT - TRANSFORMING CARE PARTNERSHIP

Who is the event for?

The event is aimed at parents and carers who have children and young people 0-25 who have a Learning Disability and/or Autism with complex mental health needs.

The primary goal of these services is to reduce the number of children and young people being admitted to hospital or reduce unnecessary or avoidable admissions for their mental health.

The Learning Disability and Autism Health Equalities Team, who work on behalf of the ICBs and Local Authorities of Southend, Essex and Thurrock (SET) and The Essex Alliance, the three Parent Carer Forums across SET Essex, would like to invite you to an Information Event on Transforming Care across SET Essex Footprint.

What is transforming Care?

Transforming care is all about improving health and care services for CYP (0-25) who have autism and/or a learning disability, so that more CYP can live in the community, with the right support, and close to home.

Within Southend, Essex and Thurrock (SET Essex) this programme of work is delivered by the Learning Disability/Autism Health Equalities Team.

The event will be an information sharing event to ensure that charities and local organisations that support families have the information needed to support their families.

The day will involve speakers from:

• LD/A Health Equalities Team: An overview of the Programme - What is Transforming Care?
• Dynamic Support Register (DSR)
• C(E)TR Team (NELFT): Care Education Treatment Reviews (C(E)TRs)
• HCRG Care Group :What is the Keyworker Service & How Can We Help?
• The All Age Autism Outreach Service (NEW!)
• Southend, Essex and Thurrock Child and Adolescent Mental Health Service (SET CAMHS)

Event Details:

Date: Tuesday 12th November 2024

Time: 9.30am welcome tea and coffee. 10am start and 2pm finish. A light lunch will be provided.

Location: The County Hotel, 29 Rainsford Road, Chelmsford CM1 2PZ

(The hotel has a car parking, and there are other local fee paying car parks nearby.

There are great transport links, with Chelmsford train and bus stations 0.2 miles from the hotel).

Event Location

We fully understand that the venue may not be local to you. Due to budgetary constraints and capacity we are unable to host 6 events across the whole of Essex (Southend, Thurrock, Mid, South, West and North Essex), providing a more local venue for all Parent and Carers.

Chelmsford provided a suitable venue and is a relatively central location within Essex, with good travel links and the venue has a car park.

We are planning further events across Essex regarding Transforming care and we welcome feedback regarding future meetings, and you can share your views with us here.
