Join us for our free, in-person Aging in Place Workshop Series — created for families and individuals who want to plan ahead with confidence.
These sessions are nurse-led and developed, grounded in real-life experience supporting people as they navigate care decisions, system challenges, and life transitions. We created these workshops because planning for the future can feel overwhelming — and too often, families are left trying to figure it out alone.
This series is for anyone who wants clarity before a crisis happens. Whether you’re supporting a loved one, thinking about your own future, or simply wanting to understand your options, these workshops are designed to provide practical guidance and real-world insight.
Our focus is simple: education, preparation, and peace of mind.
There is no pressure and no sales pitch — just honest conversation, helpful resources, and a safe space to ask the questions you may not know who else to ask.
It is not necessary to attend all 4 workshops, simply choose the topics that matter most to you. Each session is approximately 1 hour long. Please bring a pen and paper.

April 21: Workshop 1: Understanding Care Options & Funding
Making sense of what’s available — and what’s not.
Purpose:
Help participants understand the complex provincial care system and how to realistically plan for aging in place.
Topics:
The landscape of care: public, private, and community-based
How government funding works
Overview of services: home care, long-term care, respite, assisted living
Navigating assessments, waitlists, and referrals
Funding options: grants, tax credits, caregiver benefits
What to do while waiting for care and how to expedite the process
How private and community supports can fill the gaps

April 28: Workshop 2: Navigating Transitions from Hospital to Home
What to expect, what to ask, and how to prepare.
Purpose:
Support families through one of the most stressful periods — discharge from hospital — with realistic tools and system navigation tips.
Topics:
What discharge planning actually looks like (and what it often misses)
Questions to ask before a loved one comes home
Temporary vs. long-term care needs
Accessing short-term help (nursing, physio, respite- this has a few slides with our local community partners who provide services (private and non profit organizations)- we can remove this if you prefer)
When your home needs to adapt fast
Building a discharge-to-home care plan
Preventing readmission and caregiver burnout

May 4: Workshop 3: Preparing the Home for Aging in Place
Practical ways to make your home safe, comfortable, and long-term livable.
Purpose:
Teach participants how to assess and adapt the home for long-term aging with comfort, safety, and dignity.
Topics:
Fall prevention and home modifications
Bathroom, kitchen, and bedroom safety
Assistive devices and funding options
Seasonal and environmental risks
Technology to support independence
When to reassess or consider downsizing
Setting up the home after a hospital discharge

May 12: Workshop 4: Building a Support System & Having ‘The Talk’
Care takes a village — let’s talk about how to build yours.
Purpose:
Guide families through the emotional and practical conversations about aging, care preferences, and creating a circle of support.
Topics:
How to build a support network (family, friends, neighbours, professionals)
What if you don’t have local family?
Local resources for social connection, respite, peer support
Identifying your “care circle”
Having ‘The Talk’: When, how, and what to say
Discussing independence, safety, and end-of-life wishes
Getting legal and medical plans in place (POA, directives)
