Join us at the San Juan Health & Wellness Fair (Henderson Fine Arts Center 9000 rooms) on March 3, 2026, from 10 a.m. – 4 p.m. as we’ll be providing free flu and COVID vaccinations. Birth certificate replacements will be able to be printed for those born in New Mexico. The search fee is $10 for one birth certificate. Cash or money order is accepted. Identification is required and acceptable forms include driver’s license, Social Security cards, Medicaid IDs, and Tribal IDs.
*Changes to birth certificates will not be available.
We will be providing information on NMDOH services including Disease Prevention, Harm Reduction, Health Promotion, Vital Records, and School Health and Mental Health Advocacy.
