Representatives from Healthy North Coast, RACGP, ACRRM, Pharmaceutical Society of Australia, North Coast Allied Health Association and the Australia Practice Nurse Network will share their perspectives on primary care access, multidisciplinary care in general practice and aged care, chronic disease management and hospital avoidance programs.
