How to Sign up for the Pre-Participation Physical Exams

1. Click the orange button "Check Availability" and register with the student and parent/guardian's info.
2. Be sure to fill out the patient history form through the RIIL if you haven't already done so.

No Insurance Necessary. Spots are limited and registration is required. Students under 18 must be accompanied by a parent or guardian.
