(depending on the case)

REQUIREMENTS:
▪ CNA or PCA Certification
▪ Valid Driver’s License
▪ Reliable Transportation
▪ Current PPD is a PLUS
📌 Please Bring:
- Certification
- Driver’s License
- Social Security Card
- Proof of PPD within the last year
(If needed, we can assist with scheduling your PPD and Urine Drug Screen.)
