• Annual screening mammograms are

covered for patients ages 40 and over

• Mammogram will be billed to patient's

insurance company

• Financial assistance for those who qualify

• Patients under 40 are responsible for

verifying insurance coverage prior to

making appointment

Scheduled Screening Date & Time

• MammoVan will arrive 30-45 minutes

prior to event start time

• Patient must bring valid ID, insurance

card, and copies of your previous

mammograms to the appointment

• Scheduled appointments are required.
