Take exit 25 from I-490 E, Follow NY-31F W and Haffey Roadway to your destination, Turn left onto NY-31F W 0.2 mi , Turn right onto Haffey Roadway 0.2 mi, Turn left Destination will be on the left, after Haffey rd turn right on Loop Rd for Lot F to park.

Take NY-33 E/W Main St, I-490 E, I-590 S, Elmwood Ave and NY-96 S/East Ave to Haffey Roadway in Pittsford, Destination will be on the left, after Haffey rd turn right on Loop Rd for Lot F to park.

Get on I-490 W , Follow I-490 W to NY-31F W in Pittsford, Take exit 25 from I-490 W , Turn left onto NY-31F W 0.4 mi, Turn right onto Haffey Roadway 0.2 mi, Turn left for the Destination, while after Haffey rd turn right on Loop Rd for Lot F to park.

Info: Attendees will be taught on how to write (hormonal care, breast surgery, genital surgery, FFS, MFS) Letters. For Trans-binary folx's, & Gender Non-conforming & Non-binary folx's. Template letters will be given. Beyond the letter, how to document in your EHR system, and the next coordination steps for your client gaining care.

Info: Presenter will utilize powerpoint slides, worksheets, Case study examples, audience participation/ group discussion, and pre-/ post- quiz. Resources will be given in the form of articles, Sample Letters, Referral Network Forest, and handouts. There will be time allotted for questions.
