Improving Equity in Access to Palliative Care

The Grey Bruce Ontario Health Team (OHT), in collaboration with a number of local partner organizations, are working together to understand and address barriers to resources and access to care that have been created and reinforced by our systems, particularly relating to access to palliative care for Indigenous peoples and people experiencing homelessness.

This workshop has been developed with input from local providers and frontline workers in an effort to address local needs.

Topics covered will include:

Intro to IEAPC Collaborative, Indigenous social determinants of health, trauma informed care & cultural safety, Palliative care 101, Homelessness, Health Equity

Activity: Integrating a Navigator Role / Program Design
