Health care directive drop-in workshop at Burnsville Clinic

Please stop by anytime between 10am to 3:30pm for an education opportunity and assistance with completing your Health Care Directive. Patients of all ages are welcome. A Primary Care Provider within Park Nicollet or HealthPartners system is required. Materials will be provided if needed. A notary will be present after class for completed Health Care Directives. This workshop will be in person at the Park Nicollet Burnsville Clinic in the BV Superior conference room (second floor to the left).
