HomePlace Clinic is providing free sports physicals at its Ashland location on Thursday, June 25, 2026, from 9:00am-12:00pm, and again on Thursday, July 9, 2026, from 1:00-4:00pm.
Physicals will be performed for all student athletes, including elementary, middle school, and high school students, using the KHSAA physical form at no cost for both insured and uninsured individuals. Forms will be available at the clinic and must be completed by a parent or guardian. If additional forms are required, please bring them with the student. Students under the age of 18 must be accompanied by an adult.
