Walk-in, open to the public Diabetes Support Group Visits facilitated by Registered Dietician Nutritionist, Alyson Comeau and Pharmacist, Alexis DiGeronimo are available to answer questions you have about diabetes, nutrition and medication management.
Participants do not need to be a CHC patient to attend the Diabetes Group Visits.
Depending on your health insurance plan you may need a referral from your primary care provider and may be responsible for a co-payment.
