Dengie and South Woodham Ferrers Primary Care Network (working in partnership with your GP Practice) invite you to a Health and Wellbeing Clinic at Latchingdon Village Hall 1.30-4.30pm 17th March 2025.
This event is for patients of Burnham Surgery, William Fisher Medical Centre, Trinity Medical Practice and Dengie Medical Partnership only. You will be able to see a practice Paramedic for a blood pressure check and review, a Clinical Pharmacist for a medication review, a Health and Wellbeing Coach for lifestyle support and a Social Prescriber for non-clinical support. Our digital lead will also be attending offering support with the NHS app and your practice digital communications to aid in ordering your medications, booking appointments, obtaining test results and more. We will also have a number of community services as listed in our poster!
No appointment is necessary feel free to pop along any time between 1.30pm and 4.30pm.
