Due to required school documentation, appointments are required!

If a student is new to Florida, a copy of the student's vaccine records must be provided to [email protected] prior to appointment.
Student athletes who need an ECG must register and parents must consent prior to appointment through QR code provided.
Please allow 24 hours for all calls to be returned.
