About this Event

We are delighted to invite you to our Ages and Stages Event, which will provide an opportunity to launch our Multi-Agency Guide for Supporting Children and Young People Transitioning Between Services.


Developed in response to lessons learned, this guide has been designed for our workforce and partner agencies to support children and young people as they move through key stages of their journey into adulthood. The document aspires to provide staff with a comprehensive framework for supporting young people to achieve positive outcomes during periods of transition.


The guide provides a practical framework for staff to use when considering the needs and circumstances of the children and young people they work with in relation to care planning. We strongly recommend that transition planning is considered as early as possible to ensure young people receive timely and coordinated support.


By following the processes outlined within the guide, we believe we can ensure that our amazing children and young people have the support, opportunities and assistance they need to live their best lives as young adults—receiving the right support, accommodation, education and health provision at the right time.


The document will be available to internal Gateshead Council staff via Tri.X and will also be shared securely with colleagues across health and education services.


This event will bring together the services that have contributed to the development of the guide, providing an opportunity to learn more about each service, understand how they interlink, and explore how we can work together to support young people transitioning into adulthood.

Each presentation will last approximately 10–15 minutes.

Who Should Attend?

We highly encourage attendance from professionals working with:

  • Children and young people with an Education, Health and Care Plan (EHCP)
  • Children and young people who are cared for by the Local Authority
  • Children and young people with Special Educational Needs and Disabilities (SEND)
