About this Event
Welcome to the Acorns to Oak Trees Tribal Outreach Event!
Come join us at the 2nd Annual Harley's Hope Project Sonoma County Outreach Event at Sonoma County Indian Health Project. In partnership with Sonoma County Indian Health, Indian Child & Family Preservation Program, and North Bay Regional Center, this two-day event will include an opportunity for your child to receive a free developmental and/or autism evaluation, receive IEP/ 504 (Individualized Education Plan) support, learn more about child development, connect with North Bay Regional Center and other local resources, and have some fun!
Day 1: February 28th will be reserved for assessments only.
Day 2: March 1st we will be offering assessments, a storytelling video will be shared, fun activities for kids, raffles, and lunch will be provided!
We anticipate that this will be a busy 2 day event and encourage families to pre-register! We can begin the assessment process prior to the event to reduce the time you spend on the day of the event so you can enjoy your lunch and have some fun with your child while you meet our staff and partners. Once you register, we will reach out to gather some more information and can electronically send you many of the assessments beforehand. Please let us know if. you need assistance in completing questionnaires, we would be happy to help you over the phone!
Please be advised that pictures and video will be taken at the event for further outreach and Acorns promotional materials to share about our work we do. We will also be sharing basic registration information of name, age, tribe, and type of assessment with North Bay Regional Center as they fund this part of our program. We will also work with ICFPP and SCIHP on follow-up with families referred by them, in an effort to coordinate follow-up services and appointment scheduling. All assessment reports will be strictly confidential and only released to the parent/caregiver unless authorized through a release of information signed by you.
