A Transition Presentation for Supporters of Young Adults with Disabilities - Join us for a discussion about the process of transitioning from children's services to adult services and supports!
Youth and young adults encouraged to attend with their parent or guardian. All presentations are HYBRID, in-person and streamed virtually. In-person presentations are located at the Waukesha County Health and Human Services Center, 514 Riverview Ave in Waukesha.
The first half of this presentation is done by the ADRC specialist team explaining the process of transitioning from children's services to adult services. The second half of this presentation will be done by the Social Security Administration.
