Planning ahead can help ensure that your preferences are understood while easing uncertainty and worry for your family and loved ones.
Presenters Louise Kato, LCSW, and Linda Brandeis, LCSW, will draw from their extensive medical social work experience to offer practical guidance for navigating these important and often emotionally difficult conversations.
Louise Kato has more than 30 years of experience supporting patients and families in complex medical settings, with a particular focus on dialysis and kidney transplantation. Before retiring in 2022, she served as Director of Patient Quality of Life at Northwest Kidney Centers.
Linda Brandeis brings decades of medical social work experience helping patients and families navigate health care decisions, changes in care, and end-of-life planning.
Topics will include:
- Navigating today’s complex medical systems
- Understanding advance care directives
- Planning for future health and end-of-life decisions
- Communicating your wishes with family and loved ones
