Veterans Square Sports Physical

Get your student ready for sports and other school activities during our sports physical event at the Sanford Veterans Square Clinic. A sports physical visit ensures your child can safely participate all year long.
This low-cost visit includes:
• A physical exam with heart and lung checks
• Review of injury and concussion history
• A check-in about lifestyle and habits
• Certain immunizations as needed (billed through insurance)
Please bring your $50 payment and signed sports physical form. Cash or card is accepted. Parental permission is required for your child to receive a sports physical and/or immunization.

Additional services provided during your visit that are not listed above will be billed separately to your insurance. If you prefer to file insurance for your appointment, please schedule a well-child visit with your primary care provider.
