On Friday, Oct. 16 from 9:00 AM – 11:00 AM, Nevada Senior Services is hosting a community vaccine clinic at our Adult Day ReCreation Las Vegas location. Please register on our website and fill out an Immunization Consent Form to join us!
Vaccines Offered: Flu, COVID-19, Pneumonia, Shingles, RSV, Tetanus, Hepatitis
How to Register: Please register for this event on our website at https://www.nevadaseniorservices.org/events/jones-oct16 to secure your spot. Walk-ins will be accepted based on availability.
After completing the registration, you will receive an Immunization Consent Form by email. Please complete this form before your appointment.
Registering for more than one person? Please complete a separate application for each individual. Each person must have their own completed Registration Form and Immunization Consent Form.
