TALLAHASSEE FIT JAM EVENT WAIVER AND RELEASE OF LIABILITY

This waiver and release of liability form is a legal document that affects your rights. Please read it carefully before signing.

1. ACKNOWLEDGEMENT OF RISKS

I, the undersigned, acknowledge and understand that participation in the Tallahassee Fit Jam involves physical activities, fitness training, use of equipment, and other exercise-related events that may pose risks, including but not limited to:

• Physical injuries such as muscle strains, sprains, fractures, or dislocations.

• Health complications, including dehydration, heat exhaustion, or heart-related issues.

• Damage to personal property or equipment.

I understand that these risks may arise from my own actions, inactions, negligence, or from the actions or negligence of others, including but not limited to event organizers, trainers, or participants.

2. ASSUMPTION OF RISK

I voluntarily assume all risks associated with my participation in the Tallahassee Fit Jam, including any injury, illness, or damage that may occur. I confirm that I am physically fit, have no medical condition that prevents me from safely participating, and have consulted with a physician if necessary.

3. RELEASE OF LIABILITY

In consideration of being allowed to participate in the event, I agree to release, waive, and hold harmless Kingz And Queenz Fitness, its owners, employees, trainers, contractors, sponsors, volunteers, agents, and any affiliated parties (hereinafter referred to as “Releasees”) from any and all claims, demands, damages, or liabilities arising out of or related to my participation in the event.

This release includes, but is not limited to, liability for personal injury, illness, property damage, or wrongful death caused in whole or part by the negligence of the Releasees.

4. MEDIA RELEASE

I grant Kingz And Queenz Fitness permission to take photographs, videos, and other media during the Tallahassee Fit Jam. I understand that these may be used for promotional purposes, including social media, marketing materials, and other publications. I waive any right to compensation for the use of my likeness.

5. EMERGENCY MEDICAL CONSENT

In the event of a medical emergency, I authorize Kingz And Queenz Fitness and event staff to seek medical assistance on my behalf. I understand that I am responsible for any costs incurred as a result of medical treatment.

6. INDEMNIFICATION

I agree to indemnify and hold harmless the Releasees from any claims, damages, or expenses (including attorney’s fees) that may result from my participation in the Tallahassee Fit Jam.

7. FITNESS PROFESSIONAL AGREEMENT

For all fitness professionals participating in the Tallahassee Fit Jam:

• You agree to conduct your sessions in a professional manner, ensuring participant safety to the best of your ability.

• You acknowledge that Kingz And Queenz Fitness is not liable for any injury or incident resulting from your actions, instructions, or negligence during the event.

8. SEVERABILITY

If any part of this waiver is found to be invalid or unenforceable, the remaining provisions shall remain in full force and effect.

9. GOVERNING LAW

This waiver shall be governed by and construed in accordance with the laws of the State of Florida.
