If my child does have any medical conditions, allergies, or special health needs, I acknowledge that I am directly responsible for informing Project SEED Community Liaison George Corbin at (302) 379-4159 with the specific details before the event starts.

LIABILITY WAIVER & PHOTO RELEASE AGREEMENT:

I confirm that I am the parent or legal guardian of the participating minor child, and I give my permission for my child to participate in the 2026 New Castle County Project SEED 3x3 Basketball Tournament and Sparrow Run Community Day on Sept 19, 2026. As a precondition to participating, I acknowledge and assume all inherent risks of physical injury on behalf of myself and my child. I hereby release, hold harmless, and indemnify New Castle County, New Castle County Project SEED, and their elected officials, officers, agents, and employees from any and all claims, accidents, injuries, or liabilities arising from participation, to the maximum extent permitted by the laws of the State of Delaware. Furthermore, I grant permission for photos, video, and digital images of my child captured during the event to be used by New Castle County and Project SEED for promotional media without compensation. I certify that I am 18 years of age or older, that I have read and fully understand this agreement, and that my electronic selection serves as my binding legal signature.
