Join us for an informative and supportive session designed to provide resources, education and encouragement for cardiac patients and their caregivers. A multidisciplinary team, including Ph*rm*cy, dietary, case management and social work, will be available to answer questions and offer guidance.

During the sessions, you can:

• Learn strategies to help prevent hospital readmission
• Explore virtual cardiac rehabilitation options
• Hear patient testimonials and success stories
• Connect with others in a supportive environment

Date: First Thursday of every month from 10 to 11 a.m.
