Social Prescribing connects older adults (55+) to non-medical community supports to improve overall well-being. Through a referral process, individuals receive assistance from a Case Manager and Link Worker who helps identify social, emotional and non-medical needs and connects them to appropriate services.Social prescribing assists with:
Social isolation or loneliness
Mental health challenges (e.g., stress, anxiety, mild depression)
Difficulty accessing community resources
A need for support with non-medical aspects of daily living
The link worker helps individuals connect with:
Social groups, clubs, and community activities
Transportation services
Meal programs
In-Home support (basic light housekeeping, meal preparation)
Health and wellness programs (e.g., fitness, mental health support)
Financial and housing resources
Connection to Council on Aging for their lawn maintenance and snow removal program
