Get ready for an exciting new school year at Shawnee Middle School! SMS families are invited to complete re-enrollment during our convenient one-day event. Our staff will be available throughout the day to answer questions and help make the process as smooth as possible.

August 5 | 9:00 AM-3:00 PM and 4:00-7:00 PM
Please Bring:
Photo ID
Current utility bill, lease agreement, or mortgage statement for proof of residency
Any applicable legal documents regarding custody or guardianship
Proof of Tdap vaccination for all 7th grade students, if it is not already on file with the district
Residency Affidavit:
If your proof of residency is not in your name, the person whose name appears on the utility bill, lease, or mortgage statement must be present to complete a residency affidavit. This form may also be completed in advance to help speed up the re-enrollment process.
