Care finder is a free, f2f service that assists the vulnerable.
About this Event

Care Finder is designed to support people at risk of falling through the cracks in Aged Care e.g. people experiencing isolation, mental health/AOD concerns, homelessness, communication and language barriers. i.e. we are a targeted program designed to provide intensive support to the most vulnerable older people in the community who do not have the capacity to navigate the Aged Care System on their own.
