Shaping the next phase of Primary care expansion in Mississauga
About this Event
Mississauga Health, in collaboration with the Mississauga Primary Care Network, is bringing together Family Physicians and Nurse Practitioners on November 9 to help shape the next phase of team-based primary care in Mississauga.
We've heard from family physicians and primary care providers about the pressures that exist. This session is about turning that feedback into action and shaping how proposed solutions could work in practice.
Why Attend?
You will have the opportunity to:
- Hear updates on key primary care initiatives and system priorities, including a brief update from the Office of the Medical Officer of Health (Peel Public Health).
- See how previous primary care input is shaping the proposed approach.
- Explore proposed supports for providers and patients, including team-based care, specialized services, referral pathways, digital tools, and roster supports.
- Help shape how providers and patients access supports across different practice settings and local integrated primary care teams.
- Identify what should be prioritized and tested first, and what would make these supports most useful in your day-to-day practice.
- Learn about Ocean eReferral and other digital supports designed to improve connectivity and care navigation.
- Influence the next phase of primary care expansion in Mississauga.
A detailed agenda will be shared upon registration.
We look forward to your participation in helping shape the future of primary care in Mississauga.
