Shaping the Future of Aging & Cognitive Health
About this Event
SESSION PURPOSE:
This Coffee Connections session will bring together health systems, community-based organizations, and academic partners to explore what caring for our aging population looks like in Orange County.
While aging is a broad and complex topic, this conversation will focus more specifically on cognitive and brain health, prevention, and continuity of care—key areas that shape how older adults remain healthy, independent, and supported in their communities.
Participants will explore what aging in place requires in practice, including access to local resources , affordability, and coordinated systems of support. The conversation will lift up what is working locally, where gaps remain, and how to strengthen collaboration across sectors to better support older adults without working in silos.
The session will also explore how community and social systems can better support individuals and families in taking action earlier and more effectively, and will create space for cross-sector and intergenerational perspectives on how we support aging well in our community. Lastly, the session will touch on the role of research- particularly the importance of including diverse populations in research and ensuring that insights are translated into real-world practice to improve care and outcomes.
Participants will leave with practical ideas, local insights, and opportunities for connection to support ongoing work across Orange County.
SESSION OBJECTIVES:
By the end of this Coffee Connections, participants will:
- Understand key aspects of aging and older adult health, with emphasis on cognitive/brain health, prevention, and continuity of care.
- Identify practical opportunities to strengthen support for older adults through improved care delivery, prevention-focused approaches, and system coordination.
- Recognize opportunities for collaboration among health systems, community based organizations, academic institutions, and health systems to better support aging populations.
- Understand the role of research in aging and cognitive health, including the importance of engaging diverse populations and translating findings into practice. .
- Explore strategies to better align services, programs, and research with the real-world needs of diverse older adults through translational research—integrating community voices into research and translating findings into practice to inform population health management.
- Identify at least one practical idea, connection, or next step you can take to strengthen support for older adults in your work or community.
