SEL Primary / Secondary Care Interface - Patient and Staff Workshop

The South East London ICB CMO Office and the Health Innovation Network are working together on a programme of work focused on the primary and secondary care interface. We are working to develop service standards and consensus documents across multiple domains (for example discharge letters, advice and guidance etc), compile a suite of resources on the SEL ICB website, and to promote these new resources to encourage integration into clinical practice.

One of our workstreams is centred around making sure we are properly considering the patient experience.

Part of this work involves creating a patient-facing consensus document, which will contain a set of agreements between healthcare professionals and patients that:

• Helps patients understand what they can expect when moving between clinical services
• Helps healthcare professionals understand their responsibilities.
• Sets out the mutual behaviours that help patients and nhs staff build positive relationships when navigating services

We have conducted user research and focus groups with SEL patients, and have formed a series of ‘hypothesis statements’ that cover what the biggest challenges patients face when moving between services. We have held a workshop with patients, where we explored these statements in more detail, and began to create a series of 'consensus statements' that set out what good looks like across these themes.

As a final step, we want to bring staff and patients together to refine these statements, and make sure there is agreement about the statements. We also want to explore what shared behaviours help build trust and partnership between patients and NHS staff.
