Powering Healthy Communities Through Whole Person Data Exchange

We are hosting a half-day meeting on Wednesday, April 8th, as part of a multi-county initiative dedicated to advancing regional and local whole person data priorities.

Themed “Powering Healthy Communities Through Whole Person Data Exchange,” we are bringing together healthcare providers, health plans, public health leaders, payers, and community-based organizations from across the region to:

• Discuss the latest in health, social services, and behavioral health exchange
• Identify local opportunities to improve care coordination and health
• Learn from other counties and collaboratively shape solutions that reflect your community priorities

The program will begin at 11:00 a.m.(check-in will start at 10:30 a.m.). Lunch will be provided. The meeting will conclude at 3:00 p.m., after which guests are warmly invited to stay for coffee and networking from 3:00 to 4:00 p.m.
