Pop Health 2: Understanding Social Drivers of Health & Health-Related

Target Audience:

Primary Care Providers, Nurse Practitioners, Nurses, Allied Health Professionals, Public Health Providers, Healthcare Professional Students & Community Health Related Organization Representatives

Objectives:

At the end of presentation, the participant will be able to

1. Describe how “place matters” - where we live can determine how well we live and is a significant factor of life expectancy.

2. Define key concepts.

3. Identify methods for addressing social determinants of health.
