Eighth Annual Palliative Care Professional Conference, presented by the San Diego Coalition for Compassionate Care and the CSU Shiley Haynes Institute for Palliative Care at CSUSM
Our conference addresses opportunities and challenges that emerge for families and patients planning for future healthcare needs in the context of serious illness and end of life care. The program is suited to professionals across disciplines who work in palliative care, hospice care, home health care, health education, and patient advocacy. Presenters and discussion panelists will explore questions related to advance care planning, planning documents like the POLST form, surrogate decision making, home healthcare needs, the transition to assisted living, and end of life care. Speakers will focus on culturally competent care and specific challenges in caring for families and patients diagnosed with dementia and progressive cognitive illnesses. Presentations will feature practical approaches to emerging issues, hoping to strengthen the care that we provide to patients and families across disciplines, settings, and stages in the progression of ageing and illness.
