Passport Fair in Association with Alaska Federation of Natives

The U.S. Department of State is partnering with the AFN Annual Convention to host a passport fair in Anchorage, AK from Oct. 22-24.
About this Event

You are invited to join U.S. Department of State Seattle Passport Agency offering on-site passport application acceptance and answer all your passport related questions.

Appointments are required. Please make an appointment for each individual applying for a passport or passport renewal.
What to Bring

First-Time Applicants:

• Completed, printed DS-11 Form
• Citizen Evidence, photocopy (Original U.S. Birth Certificate or Naturalization Certificate)
• Valid photo ID and photocopies of front and back (Driver's License, State or Military ID Card, Etc.)
• Passport Photo (2x2, taken in the last 6 months). For requirements, check travel.state.gov/photos.
• Payment (credit or debit card, check, money order; no cash); Fees. Determine fees and payment options at travel.state.gov/passportfees.

Renewals:

• Completed (DS-82) form; DS-82 PDF (If you answer NO to any of the statements in Section A, please bring a completed DS-11 or DS-5504 depending on your circumstances)
• Most Recent Passport Book Or Card
• Passport Photo (2x2, taken in the last 6 months)
• Payment (credit or debit card, check, money order; no cash); Fees. Determine fees and payment options at travel.state.gov/passportfees.

*We cannot accept applications for Government Passports/Official Passports.
