• Professional License: CNA / HHA Certificate
• Driver’s License
• Social Security Card
• HIV Aids Certificate
• Alzheimer’s 1 Hour Certificate from Elder Affairs
• Proof of Auto Insurance
• TB Test Results / Chest X-Ray (optional)
