PHOTO/VIDEO CONSENT STATEMENT

"I understand participating in this event can include foreseen and unforeseeable risks and hazards which may expose me to injury or death. I freely and voluntarily participate in the Event with the knowledge of the danger involved and hereby agree to assume and accept all risk of injury or death. I waive and release all claims for injuries, illness, damages or loss which I may sustain from my voluntary participation in the Event including any claims against Alpha Kappa Alpha Sorority, Incorporated®,its chapters, their respective officers, agents, volunteers, members and employees (hereinafter collectively referred to as "AKA Representatives").Photo/Video Policy and Warning: Photos, live-streaming and video footage (collectively “Video Footage”) may be taken of me participating in or attending the Event. I understand by participating in the Event, I authorize the Tau Rho Omega Chapter of Alpha Kappa Alpha Sorority, Incorporated®to use the Video Footage for non-commercial purposes including social media and the Tau Rho Omega Chapter of Alpha Kappa Alpha Sorority, Incorporated® website without additional prior notice or permission and without any compensation to me. All Video Footage is the property of Alpha Kappa Alpha Sorority, Incorporated®.By registering for this Event, I have read and fully understand the above, warning of risk, assumption of risk, waiver and release of all claims, and photo/video policy and warning. If registered online, my online signature/acknowledgement will be substituted for and have the same legal effect as an original hard copy signature."
