New Hire Orientation

For your upcoming orientation at 8am you will need to bring a few things:

• You will need to email a copy of your physical if you've had one in the past year or bring in a copy to orientation. If you haven't had one in the past year, you will need to schedule one before you can attend orientation. We do not administer physicals in house. You will need to go to your doctor. If you didn't email a copy or you don't bring in a paper copy on Orientation Day, you will be sent home. Physicals are required by the state on the date of hire for all employees. You cannot do Orientation without a physical or a confirmed date for a physical appointment. All physicals can be emailed to [email protected]
• I will also need 2 forms of identification. You will need to provide me with your full social security number before orientation as it is needed for background checks. You can email it, call me with it or bring in something I can photocopy with the full number on it.
• Please bring your COVID vaccination card if you have one. If you don't have one, you will have to fill out a quick form. A COVID vaccination is not required to work here.
• Please bring in a copy of a TB test if you have received one or a chest X-Ray. If not, one will be administered during the orientation. Please email these to [email protected]
• If you have received a Dementia Training Certificate, please bring that in and you can skip the Dementia Training Course that will be held here on the Tuesday after Orientation at 8am. Please email this to [email protected]
• Please bring in a copy of your CPR Certification if you have it. This is only required for RN/LPN positions. You cannot be put on the schedule unless you have a valid CPR Certification. We hold CPR Certifications in house and they can be booked at baystatebls.com. Please email this to [email protected]
• If you are an LPN or RN, there is mandatory training on the Wednesday after Orientation at 8am.
• There will be a 30-minute lunch break, but no meal provided so please bring a lunch or you can go out for a lunch at this time.

Please send all files or attachments to the correct contact.
