By signing up for this class I consent to the following:

I understand that I will be participating in a group wellness and/or fitness program, either in person or virtually. These sessions are designed so that I can improve my strength, endurance, flexibility, balance, core strength, and overall health and wellness. I hereby voluntarily consent to a workshop lead by Powerful Physical Therapy, PLLC and BMW Rx.

I am responsible for the decision to seek this type of movement and mindfulness program that could include improvement of the physical, psychological/emotional, and environmental aspects of my illness. I recognize that the instructors do not treat any specific disease or illness during this wellness class, and they are not licensed, certified, or registered by the state as mental health care professionals. However, all staff members are certified in conducting group wellness workshops and fitness classes.

I am fully informed that this approach to health differs from, and may not be recognized by, traditional medical standards. Clients should discuss any recommendations during this workshop with their medical professional(s). I acknowledge that I have been instructed by Powerful Physical Therapy, PLLC to alert Powerful Physical Therapy, PLLC of any special needs, injuries, preferences, or considerations prior to starting the first session, as these could affect my safety and security during the group wellness and/or fitness program.

I understand that Powerful Physical Therapy, PLLC and BMW Rx reserves the right to deny treatment if it is not deemed by Powerful Physical Therapy, PLLC and BMW Rx to be in the best interest of the client or staff. I release Powerful Physical Therapy, PLLC and BMW Rx of all liabilities for my health and safety during my participation in this treatment process. I further understand by signing below that, I release all employees of Powerful Physical Therapy, PLLC and BMW Rx of personal liability. By signing below, I agree that if there are any disputes of liability that only Powerful Physical Therapy, PLLC and BMW Rx may be sued. Further, if a suit is to take place the case must be filed in an Arbitration Court of New York within one year of participating at Powerful Physical Therapy, PLLC and BMW Rx.

Testimonial/Digital Media Consent: I hereby acknowledge and authorize Powerful Physical Therapy, PLLC and BMW Rx to utilize any social media reviews, pictures, videos, or other digital content of services provided. I understand I will not be compensated for the content nor receive attribution for the content. I may withdraw authorization through specific, written rescission.

I give permission to Powerful Physical Therapy, PLLC and BMW Rx to use the contact information I have provided to contact me in regard to services provided or other health-related alternatives that may be of interest to me.
