MOVE Every HEART MKE 2026 Tour
We’re excited to announce the MOVE Every HEART Milwaukee 2026 Tour—a community-driven effort to bring heart health education, wellness resources, and free health screenings directly to neighborhoods across Milwaukee.
The Milwaukee Heart Health Learning Collaborative (MHHLC) is a partnership between the Wisconsin Department of Health Services Chronic Disease Prevention Program (CDPP), Health Connections, Inc., and Y-EAT Right. Together, we are committed to bringing community and clinical partners together to strategically leverage local assets, services, and expertise to improve cardiovascular health throughout Milwaukee.
Throughout the tour, community members will have access to valuable resources and services including:

❤️ Free Cancer Screenings
❤️ Medication Reviews
❤️ Food Demonstrations & Nutrition Education
