Presentation ~
"Practical And Ethical Challenges to Consider in the Three Major NASA Accidents: Apollo 1, and Space Shuttles Challenger, and Columbia" by Rev. William S. Wick
Practically, each accident evidenced correctable engineering and design flaws. Ethically, each accident exposed communication breakdowns (“silos”) in which the flow of critical information was hindered, overlooked, or dismissed. This along with a lack of independent oversight, inadequate safety protocols , and negligent risk assessments amid time-line pressures to maintain launch schedules to justify program costs contributed to the tragic loss of three crews and the near loss of the Apollo 13 crew.
