Longview Your Voice Your Choice Advance Care Planning seminar kickoff event

This is the kickoff event for the Your Voice Your Choice advance care planning (ACP) seminar. Where you will not only learn about advance directives but also ACP's role in your care and the care of patients at PeaceHealth.
This first event will focus on the launch of these classes, inviting both community members and staff from organizations who are interested in learning more about advance care planning.
Expect to learn about how to fill out an advance directive and the role of a POLST (portable order for life sustaining treatment) in your care and the care of patients.
