Locala Stakeholder Referral and Triage Event

Locala are Introducing a new project to create an improved referral, triage, and care coordination model.

The key aim by Spring 2027 is to introduce a person-centred referral and

triage model to achieve the following;

• New referral model based on a person needs rather than service criteria with care co-ordinators/clinical led triage empowered to convey and allocate to service teams efficiently.
• Co-ordinate and simplify the approach to referring patients to multiple service.
• Ensuring all referrals are triaged appropriately in line with agreed pathways.
• Support patients with complex needs ensuring they receive the appropriate support and care.
• Promote patient safety and efficiency: ensuring patients are referred to the correct place at the right time, minimising risks and waste and promoting referral efficiency.
• Build on feedback and utilising digital tools to support referral and triage where appropriate.

The project remains critical to achieving a streamlined, patient-centred referral and triage system, reducing duplication, improving care coordination, and supporting integrated health and social care delivery.
