Join us for a thoughtful and empowering community conversation during Advance Care Planning Month.This one-hour session is designed for community members who want to better understand how to plan ahead in ways that honor dignity, choice, and personal values. We welcome individuals with disabilities, older adults, caregivers, family members, and anyone supporting a loved one. Healthcare professionals such as nurses, social workers, case managers, and others working in the field are also encouraged to attend and deepen their understanding of advance care planning.
Whether you are planning for yourself, supporting a loved one, or working with individuals in your professional role, this program offers accessible information and practical tools to help you take the next step.
During this session, you will learn:
- What advance care planning means
- Why it is especially important in the aging and disability space
- Tools available in Maryland, including Advance Directives, Five Wishes, conversation guides, and information about the Medical Orders for Life-Sustaining Treatment (MOLST) form
- How to begin conversations with loved ones
- Where to turn for support
Planning ahead is one of the greatest ways to protect your independence and reduce stress for yourself and your loved ones.
