Let's Connect Information Sharing Session- December

Health Connections Mendip often receive request for information sharing. We have created this one hour session to share a brief background to our service with a focus on our current Let's Connect training and projects.

Anyone is welcome to attend. The presentation covers

• The background of the Health Connections Mendip service including the role of Health Connectors (Social prescribing link workers and Health Coaches).

• Information about our online directory and resource centre.

• What our key access points for information, support and opportunities are.

• An outline of the Community Connector work and the additional Connector roles such as Green and Digital Connectors.

• Details of our Let's Connect projects- such as the Talking Bench, Postcard project and Town Cooks

• How we've worked with the community to deliver a shared project.

• Time to ask and answer questions.
