• DESIGNED TO INTRODUCE 3rd- 9TH GRADERS TO TENNIS OR IMPROVE SKILLS
• TENNIS PRO REAGAN WILLMAN WILL BE LEADING OTHER TENNIS PROFESSIONALS AND HIGH SCHOOL TENNIS VOLUNTEERS
• PLAYERS CAN ATTEND AS MANY SESSIONS AS THEY WANT
• RACKETS PROVIDED, IF NEEDED
• PLAYERS WILL PLAY MATCHES IN A MINI-TOURNAMENT AT THE END OF 10 WEEKS

CLINICS ARE MONDAY AT GHS & WEDNESDAY AT EVHS : 7:00 pm-8:30 pm at High School Courts, starting June 8, 2026

BY REGISTERING YOUR CHILD YOU ARE AGREEING TO:

Liability Waiver and Release Form for a Minor

This is a release of legal rights. Please read and understand before signing.

I hereby certify that I am the adult parent or legal guardian of the minor child I am registering under the age of eighteen years. I consent to his/her participation in recreational activities at the tennis courts located at the Berry Creek Country Club, the Georgetown Country Club, the City of Georgetown tennis courts, East View High School, Georgetown High School, or any other school tennis courts in the City of Georgetown during the Nelson Ten Week Tennis /Tennispalooza program.

I understand and acknowledge that I am fully aware of and assume the risks (including but not limited to the risk of injury, serious bodily injury or death, property loss or damage, or loss of any other kind) of my minor child’s participation in recreational activities at the tennis courts and his/her use of the tennis equipment. I recognize my responsibility to ensure that said minor child participates only in those activities for which he/she has the required skills, qualifications, training and physical condition. I understand that the Nelson Tennis Foundation and/or the owners of any tennis court used during the program shall have no responsibility to pay for medical treatment and related costs if said minor child is injured.

I further understand and agree that the Nelson Tennis Foundation supplies any equipment used for tennis “as is” and that the Foundation disclaims all warranties, express or implied, including warranties of merchantability and fitness for a particular purpose.

Knowing the risks described above, I agree, personally and on behalf of the minor child named above, to assume all the risks and responsibilities surrounding my minor child’s use of the facilities and the equipment. To the fullest extent allowed by law, I hold harmless and agree to indemnify the Nelson Tennis Foundation, its officers, directors, board members, staff, volunteers, employees and agents from and against any present or future claim, cause of action, loss or liability for injury to person or property, which said minor child may suffer or for which said minor child may be liable to any other person, related to said minor child’s participation in recreational activities at the tennis courts and the use of the tennis equipment, resulting from any cause whatsoever, and regardless of fault.

I hereby grant the Foundation permission to use my child's likeness and voice in photographs, video recordings, and electronic images, without payment or any other consideration and waive any rights to publicity or rights of privacy in these images.

I am at least eighteen years of age and have carefully read and freely signed this Liability Waiver and Release Form. I understand and agree that no oral or written representation can or will alter the contents of this document. I agree that this agreement shall be governed by the laws of the State of Texas.
