The Aging Inspired Advance Care Planning Coalition and the Annville Free Library invite you to join us Monday, November 10 from 5:30-7:30pm.
Learn where to start these conversations with your children, your parents, or the most important people in your life, and your doctors. Understand what the documents are that you need and how to use them. This is a conversation often avoided in our community until it is too late to help.
Receive resources to review at home with family/friends. Understand how to pick the right people to speak for you when you cannot speak for yourself due to a health crisis.
Review and understand why updating your documents with loved ones and your health care professionals is vital as life situations change. You will receive a free copy of Five Wishes, a booklet containing both a Living Will and Power of Attorney for Healthcare.
