Location: CHDC Boyertown Office – 6 E. Philadelphia Avenue Boyertown, PA 19512
- Bring completed sports physical forms from school district.
- A parent or guardian must be present for children under age 18.
- No cost (donations accepted).
- Available to Boyertown students and children in the district.
- CALL TO SCHEDULE: 610-326-9460, extension 850 or schedule in person at the CHDC Boyertown office.
- Walk-ins are accepted based on availability.
