Free Sports Physicals

Providence Medford Sports Medicine and Rehab will be providing free sports physicals to student athletes.
Have your student athlete come dressed in shorts and a T-shirt. They will also need their school’s required sports physical examination/clearance form.
You can print your student athlete’s OSAA physical examination form on OSAA’s website.
