Ruff Haven Crisis Sheltering will be hosting a free vaccination clinic for dogs and cats, with the generous partnership of Humane Society of Utah and Vetscursions. Thank you to the Humane Society of Utah and Petco Love for sponsoring this event.
Vaccinations will be given by appointment only. Every pet will require its own appointment. Please fill out this form for each pet needing vaccination.
Included in the vaccination clinic are:
-1-year Rabies vaccine (Animals must be 16 weeks old to receive)
-1-year DHPP (core vaccine for dogs) (Must be 6 weeks old)
-1-year FVRCP (core vaccine for cats) (Must be 8 weeks old)
-Microchip
A volunteer will retrieve your pets from your car and take them in for vaccinations, unless they require assistance from the owner. All animals must be on a leash or in a carrier.
**Please note we are unable to serve community cats.

Las vacunas se administrarán únicamente con cita. Debido a las restricciones de COVID, no podemos aceptar visitas sin cita previa. Cada mascota requerirá su propia cita. Complete este formulario para cada mascota que necesite vacunación.
La clínica de vacunación incluye:
· 1-año Vacuna contra la rabia (los animales deben tener 16 semanas de edad para recibirla)
· DHPP (vacuna básica para perros) (debe tener 6 semanas de edad)
· FVRCP (vacuna básica para gatos) (debe tener 8 semanas de edad)

Un voluntario recogerá a sus mascotas de su automóvil y las llevará a vacunar, a menos que requieran la ayuda del propietario. Todos los animales deben de estar en correa o en un portador.
